Scott and I had discussed putting an order form in there actually, it would have been a free standing piece...but time constraints played a role and I'm sure there would have been an additional cost factor involved as far as the printing and insertion so we just left it as is.

I ordered mine on April 5th and still don't have mine. I still check the mailbox first thing when I get home everyday though.
