    If Herman's Hermits is disqualified then according to Wiki Small Faces formed with that name in '65, so they seem like a close second.

    Hmm. Going through the names my guess is that the earliest formed band listed so far is Herman's Hermits. They originally formed as Herman & the Hermits, but I don't think they released anything under that title.

    It drives me crazy when bands go by both. I get all OCD when I import albums into iTunes and band names come up as (for example) The Pixies or Pixies depending on the album. I always fix it.

    Captan Beefheart and HIS Magic Band or THE Magic Band also drives me nuts.
