What is the proper way of addressing a former US President ?

    It is Mr. President....that would be if you were addressing him directly. Referring to him as ex-president is appropriate when talking about him in general.

        President Carter lives about 20 miles down the road from me. I went to school with his daughter Amy and all of Billy Carter's kids. Rosalynn Carter's brother was my high school math teacher. Our school librarian was president Carter's sister-in-law. I first met him when I was in second grade, when he was governor. I know people around here who have known him his entire life and call him Jimmy. I see him around fairly often, and I've always addressed him as President Carter.
