The guy headbutting still had his helmet on hitting it into the head of the guy with out it.

Headbutts are always given using the upper forehead.
This is a very strong area of the skull because of the corner your skull meeting where the forehead bends into the top of your head.
In effect, it's like hitting someone with the corner of something.

There is a youtube video of a guy hitting a friend (at the friends request) on the forhead. Broke the hand of the guy doing the hitting.
