Notice the wardrobe error in the picture above of Kirk and Sulu. I never noticed those kind of things watching Star Trek as a kid, but found many later on watching it as an adult.Comment

I've seen that on a few early episodes in the first year, but only with CPT Kirk. I can't list the episodes or anything, but it's typically very short scenes.
I've never seen another crewmember without the insignia.

Seeing Trek in High def, you can sometimes see a slightly offset line of stitching around the insignia because it was sewn back on in a slightly different place.
The Enemy Within was the only time I could remember when a missing insignia was so prominent.
But in a Third season episode "Lights Of Zeter" Scotty spent a lot of the episode wearing a science insignia instead of his usual engineering one.Comment
