Huh, not sure. I was going to say Kiwi Kingston from Hammer's "Evil of Frankenstein", but that's not him, and it's 3 years early.
All I can find online is that it was by Basil Gogos, which you can kind of tell. I can't find anything on the subject.

DAVID HORNE solved the MYSTERY OF FM #13 in his epic work, GATHERING HORROR! On page137 of his massive Warren catalogue Horne writes:
"The cover, which recreates the image that appeared in the Monster Club application in issue 3 and 4, was uncredited, but articles about Basil Gogos in 'Horrorshow' and 'Fangoria' confirm it was his. In 'Fangoria' #138 Gogos stated that the art was actually a rough preliminary study, but that James Warren liked it as it was. (Ackerman, however, did not and has stated so.) The image originally was a person in a Frankenstein mask that Warren had photographed at local newsstands for publicity.".Comment

^Wow, interesting! I know early Famous Monsters covers did feature photos of masks, and even some paintings of them, but I didn't know this was one of them.
